Video Summary
In this AUGS webinar, Dr. Christine Vaccaro presented three mini-lectures on clitoral adhesions, testosterone therapy for low libido, and sexual tools. She reviewed clitoral anatomy, causes of clitoral pain, and how adhesions can trap keratin debris and cause burning, itching, reduced sensation, and orgasmic difficulty. She described clitoral adhesiolysis, emphasized topical anesthetic, steroid therapy, barrier ointment, and daily retraction/hygiene to reduce recurrence, especially in patients with lichen sclerosus or other inflammatory conditions.

She then discussed female sexual desire as a blend of biology and psychosocial factors. She explained the neurohormonal roles of estrogen, progesterone, dopamine, norepinephrine, melanocortins, and testosterone, and reviewed FDA-approved options for hypoactive sexual desire disorder: flibanserin and bremelanotide. The talk highlighted global consensus supporting testosterone therapy in carefully selected women with clinically bothersome low libido, using physiologic dosing, monitoring total testosterone and SHBG, and adjusting treatment to keep levels in the female range.

Finally, Dr. Vaccaro reviewed sexual tools and devices as legitimate medical aids, including vibrators, suction devices, wands, and dual-stimulation devices. She cited studies showing benefit for pelvic pain and sexual dysfunction, and encouraged clinicians to normalize sexual health conversations and device use.
