Pdf Summary
This study aimed to evaluate the incidence of complete evaluation of the urinary tract during hysterectomy for benign indications. The study found that only 22.3% of women had documentation of a complete evaluation of the urinary tract. A total of 318 women were included in the study, with 84 women having documentation of a partial assessment, and 163 women having no evaluation of the urinary tract documented. Cystoscopy, which is a method for evaluating the urinary tract, was performed in only 23% of cases. The mode of hysterectomy and the indication for hysterectomy were found to be associated with the documentation of a complete evaluation of the urinary tract. Robotic-assisted total laparoscopic hysterectomy had the highest incidence of a complete evaluation, and the indication of pelvic pain also had a higher incidence. Five injuries to the urinary tract were identified, all of which were cystotomies, and only one injury went unrecognized during the surgery.

The study concludes that there is a need for improvement in performing and documenting a complete evaluation of the urinary tract during benign hysterectomy. Urinary tract injuries during major gynecological surgery can be missed by visual inspection alone, and routine cystoscopy use has been shown to have high injury detection rates. Therefore, it is important to establish a standard of care for the evaluation of the urinary tract during hysterectomy to reduce the incidence of missed injuries.
