A retrospective cohort study was conducted to compare the postoperative outcomes, including recurrence, after transobturator tape (TOT) surgery in different age groups. The study included female patients diagnosed with stress urinary incontinence (SUI) who underwent TOT surgery from January 2013 to October 2015. The patients were divided into three groups based on age: ≤50 years (group 1), 51-69 years (group 2), and ≥70 years (group 3). The primary outcome assessed was the occurrence of postoperative complications and recurring urinary leakage symptoms. Patients with less than 1 year of follow-up and those who had previous incontinence surgery were excluded from the study.<br /><br />The prevalence of stress urinary incontinence varies by age, and the failure rate after surgery appears to increase with age. However, there are limited studies comparing the effects and failure rates after surgery between different age groups.<br /><br />The study aimed to evaluate the postoperative outcomes after TOT surgery in three age groups. The number of vaginal parity, rate of home delivery, and experience of difficult labor increased with age. Despite this, the recurrence rates were similar across all three age groups. Therefore, TOT surgery seems to be an effective treatment for stress urinary incontinence regardless of age, even in older patients with a history of home delivery or difficult labor.<br /><br />The study reviewed a total of 310 patients who underwent TOT surgery, with 281 women available for analysis at the 1-year follow-up. There were 78 patients in group 1, 154 in group 2, and 49 in group 3. The postoperative outcomes, including urinary symptoms and complications, were compared among the three age groups.<br /><br />The results showed no significant differences in weight, BMI, preoperative hormone therapy, or previous hysterectomy between the groups. However, the number of pregnancies, parity, vaginal parity, menopause, rate of home delivery, and experience of difficult labor significantly increased with age.<br /><br />Patients in all age groups reported a significant improvement in their condition after TOT surgery. The most common postoperative urinary symptom was frequency, with a significant difference observed between the groups. Urine leakage, which can be considered a symptom of recurring urinary incontinence, occurred in a small percentage of patients in each group with no significant difference. Additionally, the rate of complications did not show a significant difference between the age groups.<br /><br />In conclusion, TOT surgery appears to be an effective treatment for stress urinary incontinence across all age groups. Further studies with a larger subgroup of women and longer follow-up periods are necessary to validate these findings.
