A study was conducted to compare the outcomes of mid-urethral sling (MUS) procedures in women with and without diabetes mellitus (DM). The Trial of Mid-Urethral Slings (TOMUS) was a randomized trial that assessed subjective and objective success rates for urinary incontinence (UI) in women who underwent retropubic and transobturator MUS procedures. The secondary aims were to compare the resolution of overall and stress-specific UI, morbidity, time to adequate voiding, satisfaction, and quality of life in the two groups.<br /><br />The study found that there were differences in voiding symptoms pre-operatively that may be related to DM. Women with DM planning MUS surgery should be informed about the higher risk of leakage with activity following the surgery. However, there were no significant differences in treatment success and post-void residuals between women with and without DM after the surgery. The study concluded that MUS remains a safe and effective surgical option for the treatment of stress urinary incontinence (SUI) in women with DM.<br /><br />The study also examined the effect of DM on post-void residual (PVR) and lower urinary tract symptoms after MUS placement. The baseline demographics and symptom scores of 597 subjects with and without DM were compared. The study found no significant differences in PVR and change in PVR at 2 and 6 weeks post-op between the two groups. Similarly, there were no differences in treatment success between women with and without DM. However, post-operative patients with DM were more likely to report leakage related to physical activity.<br /><br />The study suggests that the differences in preoperative symptoms between women with and without DM may be related to the effect of DM on sensory and voiding function. It is important to note that the study had limitations, including the fact that the patients included with DM seemed to have mild disease, as over half were treated only with diet.<br /><br />Overall, the study demonstrates that MUS is a safe and effective surgical option for women with DM who are experiencing SUI. However, healthcare providers should counsel women with DM regarding the risk of leakage with activity following the surgery.
