This preliminary study examines the effect of an additional surgery, specifically synthetic midurethral sling (TVT), on overactive bladder (OAB) symptoms in the surgical repair of pelvic organ prolapse (POP). The study aimed to determine if adding TVT during prolapse repair improves OAB symptoms.<br /><br />The study involved 40 patients with ≥ stage 2 POP and OAB symptoms, divided into two groups: prolapse repair alone (PR) and prolapse repair with TVT (PR+TVT). Both groups showed improvement in OAB symptoms and quality of life (QOL) 6 weeks post-op, with a more significant improvement in the PR+TVT group.<br /><br />At 6 months post-op, both groups still showed improvement in OAB symptoms compared to baseline. However, the PR group continued to improve, while the PR+TVT group experienced an increase in OAB symptoms between 6 weeks and 6 months.<br /><br />Although this study had a small sample size and further research is needed, the results suggest that both prolapse repair alone and with concomitant TVT can improve OAB symptoms. However, there was no significant difference between the two groups at 6 months follow-up.<br /><br />The study acknowledges the controversial nature of additional anti-incontinence surgery in treating POP and the need for larger population studies to draw definitive conclusions.<br /><br />Overall, this study provides preliminary evidence that adding TVT to prolapse repair may offer initial greater reduction in OAB symptoms and improvement in QOL measures. However, further research is required to confirm these findings and determine the long-term effects of this additional surgery on OAB symptoms.
