This systematic review aimed to evaluate the safety and efficacy of different methods for intraoperative evaluation of urinary tract injuries during pelvic surgery compared to the use of indigo carmine. The review included studies published up until October 2016 and searched multiple databases. <br /><br />The review identified a total of 11 methods for urinary tract assessment during pelvic surgery, including various types of cystoscopy (with normal saline, dextrose, or "not otherwise specified"), intravenous (IV) injections of indigo carmine, sodium fluorescein, or methylene blue, IV furosemide, oral phenazopyridine or vitamin B, prophylactic preoperative retrograde ureteral stent placement, and transabdominal color doppler ultrasound.<br /><br />Overall, the study concluded that all evaluated methods were safe and effective, with specificity and negative predictive value (NPV) greater than 99%. The rates of complications associated with these methods were similar to those observed with indigo carmine.<br /><br />However, the authors noted that larger studies are needed to evaluate the diagnostic accuracy of some newly described methods. <br /><br />The study selection process followed the PRISMA and MOOSE guidelines, and there were no restrictions on study design or languages. The review included effectiveness and complications as outcome variables and excluded studies that focused on urinary tract anomalies, stones, or malignancies, as well as those that used selective rather than routine evaluation methods.<br /><br />In conclusion, this systematic review provided a comprehensive compilation of available evidence on the intraoperative evaluation of urinary tract injuries during pelvic surgery. It found that various methods were safe and effective, with comparable performance to indigo carmine. However, further research is needed to assess the diagnostic accuracy of newer methods.
