This research article discusses the use of Surgicel, an oxidizing regenerating cellulose gauze, in the prevention of recurrent labial adhesions (LA) in postmenopausal women with lichen sclerosus (LS). LS is a chronic inflammatory skin condition that can lead to various long-term complications including chronic irritation, urogenital symptoms, and an increased risk of squamous cell carcinoma.<br /><br />The study involved two postmenopausal patients with refractory LS who underwent surgical management for symptomatic LA. In both cases, the patients had previously failed medical management. The surgical approach involved the use of Surgicel intraoperatively. Sharp and blunt dissection were performed to restore the anatomy, resulting in raw surfaces in the dissected beds. Surgicel was then sutured in place to cover all exposed surface areas to prevent early reformation of adhesions.<br /><br />The procedure was well-tolerated by both patients, and no intra-operative or post-operative complications were reported. Surgicel completely resolved and disappeared by postoperative day 7. No signs or symptoms of recurrent LA were noted during postoperative visits.<br /><br />The authors conclude that the use of Surgicel in these two postmenopausal patients prevented the recurrence of LA for at least 3 months and 9 months, respectively. The addition of Surgicel did not increase the risk of complications or infections. The patients were advised to continue using topical estrogen after the procedure.<br /><br />Overall, this study suggests that Surgicel may be an effective intervention in preventing recurrent labial adhesions in postmenopausal women with lichen sclerosus.
