Pdf Summary
The study aimed to identify barriers to the universal use of cystoscopy during hysterectomy and find potential solutions to overcome these barriers. The researchers distributed a questionnaire to 18 gynecologic surgeons and conducted focus group sessions to gather qualitative data. The questionnaire revealed that increased operative time and cost were noted as barriers to performing universal cystoscopy. The focus group sessions identified several themes, including cost, time, and training associated with cystoscopy use. Other indirect themes included the average skill in performing diagnostic cystoscopy, lack of confidence in troubleshooting cystoscopy issues, and concern for providing quality resident teaching during cystoscopy. A word cloud created from the focus group session of a private hospital showed a major theme difference related to the issue of cost compared to a low-resource setting. The researchers concluded that gynecologic surgeons at their teaching hospitals did not employ universal cystoscopy due to lack of confidence in troubleshooting cystoscopy issues and teaching the procedure to junior learners. They proposed a "ground-up" approach, involving e-learning, simulation, and proctored operating room sessions to address these concerns. The study highlighted the importance of intraoperative detection of urinary tract injuries and the potential benefits of routine cystoscopy during hysterectomy. The references cited studies that support the significance of cystoscopy in reducing morbidity and improving outcomes during gynecologic surgery.
