This study investigated the potential role of the urinary microbiome in Mixed Urinary Incontinence (MUI) in women. The etiology of MUI is currently unknown. The study aimed to compare the urinary microbiome in women with MUI to similarly aged controls. The primary objective was to examine whether there was a difference in Lactobacillus predominance between cases and controls. The secondary objective was to assess the composition of the bacterial community in both groups, including subsets of pre-menopausal (<51 years) and post-menopausal (≥51 years) women. <br /><br />This was a multi-center observational study that included MUI women as cases and similarly aged controls. Catheterized urine samples were collected and the v4-6 region of the 16S ribosomal RNA gene was sequenced to evaluate the urinary microbiome at the genus level. The urinary taxa were categorized into different "community-types" using Dirichlet multinomial mixture methodology. Regression methods were then used to compare the bacterial community-types between cases and controls, as well as demographic and clinical characteristics.<br /><br />The results showed that women under the age of 51 with MUI had a different urinary microbiome compared to controls, whereas no difference was found in women aged 51 and above. Women under 51 with MUI had a lower prevalence of Lactobacillus and their bacterial communities differed from controls. It remains to be determined whether the shift in MUI-associated bacteria or the preponderance of Lactobacillus contributes to symptoms. <br /><br />Overall, this study suggests that there may be a relationship between the urinary microbiome and MUI in women under the age of 51, but further research is needed to fully understand this relationship and its implications for treatment and management of MUI.
