This study reanalyzes the data from the Pelvic Organ Support Study (POSST) to define normal apical values in female patients without symptoms of pelvic organ prolapse. The POSST is a database of 1011 women who completed a prolapse symptom questionnaire and underwent a POP-Q examination. In this study, 59 women who met the definition of prolapse and 4 women who were missing a POP-Q point were excluded from the analysis, resulting in a total of 948 subjects. <br /><br />The results show that the median values for the anterior and posterior vaginal POP-Q points (Aa, Ba, Ap, Bp) are -2 cm, with an intra-quartile range of 0 to -3 cm, which suggests that values at or above the hymen are considered normal. Total vaginal length was shorter in women who had a hysterectomy, which is consistent with other literature. <br /><br />For cervical support (POP-Q point C cervix), the median value is -6 cm, but the normal range suggests it can be between -4 cm and -8 cm. In subjects who have had a hysterectomy, the apical POP-Q point C (vaginal cuff) has a median of -7 cm, with an interquartile range between -4.5 cm and -9.5 cm. <br /><br />These findings may help define success after apical repair procedures. However, it should be noted that there is no agreement on what represents normal apical support, as shown in a recent systematic review. Further research may be needed to establish a more universally accepted definition.
