Interstitial cystitis/painful bladder syndrome (IC/PBS) is a chronic condition that affects a significant percentage of women in the United States. Hydrodistention is a commonly used treatment for IC/PBS, but it is limited by immediate postoperative bladder pain and its short duration of action. Preemptive pudendal nerve block has been proposed as a way to reduce postoperative pain after hydrodistention. <br /><br />This study aimed to evaluate the effectiveness of preemptive pudendal nerve block in reducing postoperative pain after hydrodistention in women with IC/PBS. Participants were randomized to receive either 1% lidocaine pudendal nerve block or a normal saline injection. Pain scores were recorded immediately after the procedure and at 2, 6, and 12 weeks postoperatively.<br /><br />The results showed a trend towards improvement in pain scores at 2 and 6 weeks postoperatively in the group that received preemptive nerve block. However, there was no significant difference in pain scores immediately after the procedure or at 12 weeks. The study had a small sample size, so further analysis with a larger sample is needed to determine the specific symptoms that may be benefited by this treatment.<br /><br />Recruitment for the study is still ongoing, and many women expressed reluctance to enroll due to fear of being randomized to the saline arm. Baseline demographic and medical information, as well as visual analog scores and questionnaires related to pelvic pain, urgency, frequency, and interstitial cystitis symptoms, were collected from the participants.<br /><br />The study provides preliminary evidence that preemptive pudendal nerve block may be a useful adjunct for subacute pain relief in women with IC/PBS. However, further research with a larger sample size is needed to confirm these findings and determine the full extent of its benefits.
