A study was conducted to analyze the predictors of success at 5 years post-implant of the InterStim system for overactive bladder (OAB) using the InSite post-approval study. The study included subjects with OAB symptoms who had not exhausted all medication options. The purpose of the analysis was to determine the effect of baseline characteristics and response during test stimulation on the 5-year therapeutic success of sacral neuromodulation (SNM) with the InterStim system. <br /><br />For subjects with urinary urge incontinence (UI), the study found that test stimulation response was not a significant predictor of success. However, subjects were less likely to have therapeutic success if they had concomitant OAB medication use or a longer duration since diagnosis. No other baseline characteristics showed a relationship with UI therapeutic response. <br /><br />For subjects with urgency-frequency (UF), returning to normal voiding at test stimulation was the only significant predictor of therapeutic success. No relationship was observed between concomitant OAB medication or other baseline characteristics and UF therapeutic response. <br /><br />The study was sponsored by Medtronic, the manufacturer of the InterStim system. Overall, the study concluded that test stimulation success was not a predictor of 5-year success for UI subjects, but returning to normal voids after test stimulation was a predictor for UF subjects. However, the study acknowledges limitations in retrospectively exploring test stimulation as a predictor of therapeutic success. <br /><br />In summary, the study found that for subjects with OAB, the predictors of success at 5 years post-implant of the InterStim system varied. The study highlights the importance of considering baseline characteristics and response during test stimulation when predicting therapeutic success in OAB patients.
