This study aimed to identify predictive factors for higher postoperative pain levels following vaginal reconstructive surgery. The researchers conducted a retrospective descriptive study using previously collected data from six approved studies involving subjects who had undergone vaginal intraperitoneal apical suspension for pelvic organ prolapse. The study included 364 subjects with a median age of 60 years and a median BMI of 27.8. The majority of the subjects were Caucasian and had Stage II or III prolapse. The median postoperative pain score was 35mm on a 100mm scale. <br /><br />The study found that age, depression, tobacco use, and exclusion of a concomitant suburethral sling procedure were significant predictive factors for higher postoperative pain scores. Patients under the age of 60 had significantly higher pain scores, while there was no significant relationship between BMI and pain scores. Patients who used pain medication or tobacco prior to surgery also had higher pain scores. Surprisingly, patients who underwent a concomitant suburethral sling had lower pain scores. Although there was a trend, the relationship between depression and preexisting use of anxiety or depression medication did not reach statistical significance.<br /><br />Linear regression analysis confirmed the significant relationships between postoperative pain scores and age, pain medication use, tobacco use, and concomitant sling. After controlling for other potential confounding variables, age, depression, tobacco use, and concomitant sling remained as significant independent factors predictive of postoperative pain scores.<br /><br />The findings suggest that healthcare providers should consider these predictive factors when managing postoperative pain in patients undergoing vaginal reconstructive surgery. Further research is needed to explore the relationships between surgical time, preexisting pain medication use, and postoperative pain scores, as well as the impact of concomitant suburethral slings.
