Abdominal sacrocolpopexy with rectus fascia autograft is a surgical repair option for pelvic organ prolapse that avoids the use of mesh, which is associated with complications. This study aimed to report on the outcomes of women undergoing this procedure. The study included 81 patients who underwent abdominal sacrocolpopexy with rectus fascia autograft, performed by a single surgeon. The median follow-up time was 10.5 months, with the longest follow-up period being 54 months. <br /><br />The study found that the procedure yielded good anatomic outcomes, with a median prolapse exam grade of 2 for all compartments preoperatively. Five patients (6.2%) experienced postoperative anatomic recurrence of grade 2 or greater, and two of these patients required additional treatment. Four patients (4.9%) developed incisional hernias during the 5-year period, and two of them required surgical repair. <br /><br />The study highlights the effectiveness of abdominal sacrocolpopexy with rectus fascia autograft in addressing vaginal vault prolapse without the use of mesh. However, further research is needed to assess long-term outcomes and compare open and minimally invasive approaches. Mesh complications in female reconstructive surgery have become a concern, so it is important to explore alternative modalities that are safe and effective. Abdominal sacrocolpopexy is considered the "gold standard" surgical repair for pelvic organ prolapse, and the minimally invasive approach is common. The study contributes to expanding the options available for the treatment of pelvic organ prolapse.
