Analysis of Clinical Predictive Factors of ALPP <60cm H2O in Patients with Stress Urinary Incontinence in the Mexican Population - Cynthia I Infante Melendez

This study aimed to determine predictive clinical factors for abdominal leak pressure point (ALPP) < 60 cm H2O in patients with stress urinary incontinence (SUI). The study involved a retrospective analysis of 84 patients with SUI who underwent urodynamic study at the Civil Hospital of Guadalajara. The patients were classified into three groups based on the ALPP value: >120 cm H2O, between 60 cm and 120 cm H2O, and <60 cm H2O. Various clinical factors were evaluated, including age, menopause, history of anti-incontinence surgery, hysterectomy, severity of pelvic organ prolapse, absence of urethral hypermobility, and positive empty bladder test.<br /><br />The results of the univariate analysis showed that ALPP < 60 cm H2O was associated with menopause, history of anti-incontinence surgery and hysterectomy, fixed urethra, and positive empty bladder test. In the multivariate analysis, the absence of urethral mobility and positive empty bladder test were identified as independent predictors of ALPP < 60 cm H2O.<br /><br />These findings suggest that in patients with SUI, the absence of urethral hypermobility and a positive empty bladder test can predict a lower abdominal leak pressure point. This information can be useful in identifying patients at higher risk of treatment failure and inform treatment decisions. Further research is needed to validate these findings and determine their clinical implications.
