Audit of Hospitalization Costs of Abdominal, Laparoscopic and Vaginal Hysterectomy for Benign Gynecological Disease in a Tertiary Hospital - Nur Azleen Sidek, MBBS

This study compared the cost differences between abdominal, laparoscopic, and vaginal hysterectomies for benign gynecological disease in a tertiary university hospital. The researchers analyzed data from January 2016 to December 2018, identifying patients who underwent these procedures and dividing them into three groups: total abdominal hysterectomy (TAH), total laparoscopic hysterectomy (TLH), and total vaginal hysterectomy (TVH). <br /><br />The study found that of the 366 women who underwent a hysterectomy, the majority (63%) had a TAH, followed by TVH (18.6%) and TLH (18.4%). The overall hospitalization cost was the lowest for TVH at $4950, followed by $6314 for TAH and $6664 for TLH. The mean admission cost was lowest for TVH at $2895. The mean operative cost was similar for TAH and TVH ($1940 and $2055 respectively), and highest for TLH at $2362. The average length of hospitalization stay was longest for TAH at 4.2 days, and similar at 3.2 and 3.0 days for TVH and TLH respectively.<br /><br />Based on these findings, the authors concluded that TVH has the lowest overall hospitalization costs among the three modes of hysterectomy, with a similar hospitalization stay to TLH. Therefore, TVH should be considered as the surgical approach of choice if other clinical factors support its safety and feasibility for patients.<br /><br />The study suggests that further research is needed to include outcomes such as operative complications and recovery time in order to determine the overall cost effectiveness of the different modes of hysterectomy.
