Botulinum Toxin Injections for Treatment of Pelvic Floor Myofascial Pain: A Systematic Review of Published Techniques - Melanie R. Meister, MD, MSc

This systematic review aimed to evaluate the methods of transvaginal botulinum toxin injection for the treatment of pelvic floor myofascial pain in women. The authors conducted a comprehensive literature search and identified 15 studies that met the inclusion criteria. The included studies consisted of one randomized controlled trial, seven prospective cohorts, one retrospective cohort, and six case reports/case series.<br /><br />The studies varied in terms of the location of botulinum toxin injections, with five being administered in the operating room, two in the outpatient clinic, and the remaining eight not describing the treatment location. Anesthesia methods also varied, with sedation being the most common approach. The total dose of botulinum toxin ranged from 20 to 300 international units (IU), with 10-50 IU administered per injection site. The methods used to identify the injection location included digital palpation, electromyography (EMG), ultrasound-guidance, and fluoroscopy.<br /><br />Reported complications varied among the studies, with seven studies not reporting whether complications occurred. Despite the limited data available, botulinum toxin injections to the pelvic floor muscles were shown to be an effective treatment for pelvic floor myofascial pain in women. However, an evidence-based protocol for the administration of this treatment does not currently exist.<br /><br />The authors concluded that future studies should focus on optimizing the protocol for transvaginal botulinum toxin injections in order to maximize efficacy and minimize risks. Overall, the findings suggest that botulinum toxin injections have the potential to be a promising treatment option for pelvic floor myofascial pain in women, but further research is needed to establish standardized protocols for its administration.
