The study examined the relationship between bladder outflow obstruction (BOO) and detrusor overactivity (DO) in both men and women. The researchers re-evaluated urodynamic studies of 1935 individuals with non-neurogenic lower urinary tract symptoms (LUTS). The participants were grouped based on the grade of BOO, and the study compared men and women with the same classification. <br /><br />The results showed that as the grade of BOO increased, the bladder capacity decreased and the pressures increased at first sensation, normal desire, and strong desire to void. This pattern was observed in both men and women. <br /><br />It was noted that while the prevalence of DO is high in women, the prevalence of BOO is much lower in women compared to men over 45 years of age. This suggests that there may be another mechanism causing DO in women, apart from BOO. The study questions the relevance of animal studies on BOO for understanding the pathophysiology of DO in women and suggests that inflammation models may better represent the condition in women. <br /><br />In conclusion, the study found that standardized sensation mean volumes decrease and pressures increase with an increasing grade of bladder outflow obstruction in both men and women. It also showed that an increase in bladder outflow resistance is associated with a higher likelihood of detrusor overactivity in the filling phase for both genders.
