Do Patients with Positive Preoperative Testing for Stress Urinary Incontinence Regret Declining a Midurethral Sling at Time of Apical Suspension? - Sofiya Chernyak, MD

This study aimed to assess whether patients regret declining midurethral sling placement at the time of apical suspension surgery for stress urinary incontinence (SUI). The primary outcome measure was regret, and secondary outcomes included satisfaction with surgery and the presence of bothersome urogenital symptoms. The study was conducted through a chart review of patients who had undergone apical suspension surgery between 2012 and 2017. Demographic information and preoperative Urogenital Distress Inventory-6 (UDI-6) scores were collected. Phone interviews were then conducted, during which patients completed regret and satisfaction questionnaires and the UDI-6 survey. Descriptive statistical analysis was performed on the collected data. <br /><br />Of the 589 charts reviewed, 553 patients were excluded, and 36 patients met the inclusion criteria. Out of these 36 patients, 24 completed the questionnaires and 12 were unable to be contacted. The mean age of the patients was 69.5 years, with an average parity of 3.0. The majority of patients had stage 3 prolapse (41.7%), while 37.5% had stage 2 prolapse and 20.8% had stage 4 prolapse. <br /><br />The study found that patients did not regret forgoing sling placement at the time of their apical suspension surgery. Overall, patients reported being satisfied with their surgery. The UDI-6 scores showed improvement between the preoperative and postoperative periods. The low regret may be attributed to counseling, suggesting that preoperative counseling for asymptomatic patients with occult SUI should include a discussion about overall satisfaction with surgery despite not opting for sling placement.
