Does the Type of Pelvic Floor Disorder Impact Compliance with Prescribed Pelvic Floor Physical Therapy? - Patricia J. Mwesigwa, MD

This study aimed to determine if there is a relationship between the type of pelvic floor disorder (PFD) and compliance with pelvic floor physical therapy (PFPT). The researchers reviewed referrals to PFPT at an academic medical center from January 1, 2018, to June 30, 2018, and identified patients using ICD-10 codes. Initiation was defined as attending an appointment, and completion was defined as finishing a pre-planned number of sessions or terminating therapy due to improvement.<br /><br />The results showed that only half of the patients referred to PFPT initiated therapy, and only 10% completed the recommended sessions. The most common diagnoses for PFPT referral were urinary incontinence (UI) and pelvic pain. Patients who contacted for an appointment were likely to attend their first visit.<br /><br />There were significant differences in age, parity, and comorbidities between different primary diagnoses. Patients with pelvic pain were the most compliant, completing PFPT 11% of the time. Younger patients with pelvic pain were more likely to contact therapists but not necessarily more likely to complete treatment. Prior experience with PFPT did not lead to a higher completion rate in any category.<br /><br />The study concluded that providers may consider encouraging initiation to groups known to be noncompliant, such as those with anal incontinence (AI) and interstitial cystitis/painful bladder syndrome (IC/PBS). Further research is needed to identify biases and barriers to initiation and completion of prescribed PFPT.<br /><br />In summary, this study found that patients with pelvic floor disorders are often referred to PFPT, but only half initiate therapy, and the completion rate is low. The type of pelvic floor disorder may impact compliance with prescribed PFPT, and certain factors like age and comorbidities can influence initiation and completion rates.
