Healthcare Cost and Resource Utilization Associated with use of Laparoscopy with Vaginal Hysterectomy: 5-Year Single Institutional Study of 697 Patients - Emad A Elsamadicy, MD, MSc

In a study conducted at Vanderbilt University Medical Center, the healthcare cost and resource utilization associated with laparoscopy with vaginal hysterectomy (LAVH) were examined. The study included 697 adult women who underwent elective vaginal hysterectomy between 2010 and 2015. Of these, 132 underwent LAVH and 565 underwent total vaginal hysterectomy (TVH).<br /><br />The study found that there was a significant difference in mean hospital costs between the LAVH and TVH cohorts. The LAVH cohort had a greater mean cost of $34,749.5 compared to the TVH cohort, which had a mean cost of $30,294.8.<br /><br />There were also significant differences in insurance status between the two cohorts. The LAVH cohort had a greater proportion of patients with Medicaid (24.2% vs. 11.0%) and a lesser proportion with Medicare (10.6% vs. 26.0%) compared to the TVH cohort.<br /><br />The study highlights that there are significant cost differences associated with the use of laparoscopy for vaginal hysterectomies. It also suggests that there may be disparities in the type of patients undergoing LAVH compared to TVH based on age and insurance status.<br /><br />The authors note that further studies are needed to better understand the clinical indications for LAVH and to help inform decision-making regarding the most beneficial and cost-effective surgical approach.
