Iliococcygeus Fixation Compared with Abdominal Sacral colpopexy to Treat Uterine Prolapse for Elderly Patients - Yun Jin Park

A study was conducted to compare the effectiveness and safety of iliococcygeus fixation (ICG) and abdominal sacral colpopexy (ASC) in treating uterine prolapse in elderly patients. The aim of the study was to investigate the clinical outcomes of both procedures according to age. The medical records of patients who underwent either ICG with vaginal hysterectomy or ASC with abdominal hysterectomy were reviewed. The patients were grouped into three age groups: ≤50 years, 51 to 69 years, and ≥70 years. The primary outcome of the study was the recurrence of pelvic organ prolapse (POP) after surgery, while the secondary outcomes included hospital stay, duration of operation, blood loss, and complications. <br /><br />The study included a total of 166 patients, with 105 undergoing ASC and 61 undergoing ICG. The oldest age group (≥70 years) had higher parity and a greater proportion of comorbidities. The proportion of patients who underwent ICG increased with age. There was no significant difference in the recurrence rate of POP between the ICG and ASC groups. In the ICG group, there was no difference in the POP relapse rate one year after surgery among the different age groups. The ASC group had a longer operation time compared to the ICG group. In the oldest age group, patients who underwent ICG had smaller bleeding loss during the operation and shorter hospital stays compared to those who underwent ASC. The rate of postoperative complications after ICG in the oldest age group was 0%. <br /><br />In conclusion, regardless of age, ICG is an equivalent procedure to ASC in terms of the treatment outcome for uterine prolapse. Additionally, in elderly patients, ICG with vaginal hysterectomy is considered a safer surgery than ASC with abdominal hysterectomy.
