Implementation of a Multifaceted Intervention in Support of Transvaginal Hysterectomy (TVH) in an Academic Center and Its Effect on TVH Rates- Shefali Sharma, MD

This study aimed to increase the rates of transvaginal hysterectomy (TVH) at an academic center through the implementation of a multifaceted intervention. The results showed that resident performance significantly improved after a workshop using vaginal hysterectomy models. The main barriers identified by faculty were technical difficulties and lack of an experienced assistant. The TVH group, compared to the non-TVH group, was found to be older, have a lower body mass index, higher parity, lower uterine weight, and less prior abdominal surgery. Simulation models and pairing physicians with experienced vaginal surgeons were well-received strategies. The study intends to continue the interventions and track TVH rates over a two-year period. <br /><br />The study emphasizes the importance of TVH as the preferred route of hysterectomy when feasible. It suggests that improving physician skill is a modifiable factor that can lead to an increased number of gynecologists offering TVH. The intervention involved a resident workshop with vaginal hysterectomy models, distribution of faculty surveys to determine barriers, and analysis of pretests and posttests from residents. Baseline data on benign hysterectomy from December 2015 to December 2017 was collected.
