Intraperitoneal Colpopexy with Complete Vaginal Reconstruction: A Retrospective Case Series - Joseph Panza, MD

Pelvic organ prolapse is a common condition in women that can cause significant discomfort and decrease quality of life. Surgical procedures, such as uterosacral ligament suspension, have been developed to improve outcomes and minimize complications. In this retrospective study, 62 cases of uterosacral ligament suspension combined with complete vaginal reconstruction were reviewed to evaluate long-term outcomes. The study found low rates of intra-operative and post-operative complications, including ureteral injury and post-operative urinary tract infections. No bowel or bladder injuries were observed. Improvement in quality of life, including urinary and bowel function, was seen after surgery. Sexual function was maintained in most patients who were sexually active prior to surgery. The study also showed a low rate of recurrent prolapse, indicating the effectiveness of the procedure. The surgery had a low risk of complications and can be combined with a sling procedure without increased risk. The study concluded that native tissue prolapse repair with uterosacral ligament suspension and complete vaginal reconstruction is a well-tolerated surgery with good long-term outcomes. The demographic data showed that the average age of patients undergoing the procedure was 64.97 years, with a BMI of 27.52 kg/m2 and a parity of 2.45. The majority of patients were post-menopausal and Caucasian. This study provides evidence for the safety and efficacy of this surgical approach in treating pelvic organ prolapse.
