Obstructed Defecation Symptom Severity and Degree of Changes in Rectovaginal Septum Length as Detected by Dynamic Ultrasound - Ghazaleh Rostami Nia, MD, MSc

The study aimed to investigate the association between the severity of obstructed defecation symptoms (ODS) and changes in the length of the rectovaginal septum as detected by dynamic pelvic floor ultrasound. A total of 121 women with ODS were recruited for the study, and they underwent interviews, pelvic exams, and pelvic floor ultrasound. The patients were categorized into different groups based on their diagnostic results.<br /><br />The results showed that there were no significant differences in age, parity, BMI, or stage of rectocele among the groups. However, the dynamic ultrasound measurements revealed a higher compression ratio in women with severe ODS. The risk of having ODS was found to be four times greater among those with a high compression ratio (≥20) compared to those with a low compression ratio, after controlling for age, BMI, and LPDA.<br /><br />The findings suggest that women with more severe ODS experience greater changes in the length of the rectovaginal septum when intraabdominal pressure increases. This is observed as a sliding rectum in dynamic pelvic floor ultrasound.<br /><br />In conclusion, this study provides further evidence of the association between ODS severity and changes in the rectovaginal septum length detected by dynamic ultrasound. These findings contribute to the understanding of structural defects associated with ODS and may have implications for future diagnostic and treatment approaches.
