The study aimed to determine the incidence of occult malignancy at the time of sacrocolpopexy (SCP) with concurrent hysterectomy. The researchers conducted a retrospective chart review of patients who underwent SCP at a tertiary care referral center from January 2004 to June 2016. They collected demographic and perioperative data from the electronic medical record, reviewed operative and pathology reports, and collected data on the mode of concurrent hysterectomy before and after the 2014 FDA power morcellation safety communication.<br /><br />Before the safety communication, laparoscopic hysterectomy was the primary mode (41.3%), but two years after the communication, vaginal hysterectomy became the primary mode (60.7%). Before the morcellation ban, most uteri (53.1%) were removed with a power morcellator without a bag. After the communication, most uteri (64.3%) were removed vaginally or via laparotomy without morcellation.<br /><br />The study included 709 patients who underwent SCP, and 29.2% had a hysterectomy at the same time. The overall incidence of occult uterine malignancy was 1%, with one case of ovarian malignancy. No cases of delayed gynecologic cancers were identified. There were no significant differences in rates of obesity, menopause, hormone replacement use, tobacco use, or diabetes between those with or without occult malignancy.<br /><br />In conclusion, the study found that the incidence of occult uterine malignancy at the time of hysterectomy and sacrocolpopexy was 1%. The mode of concurrent hysterectomy changed significantly after the 2014 FDA safety communication.
