Pelvic Organ Prolapse in Postmenopausal Women attending a Urogynecology Clinic: Symptoms, Staging and Treatment - Sirirat Saritapirak

This study aimed to examine the symptoms, stage, and treatment options for pelvic organ prolapse in postmenopausal women attending a urogynecology clinic. A total of 408 women with pelvic organ prolapse were included in the study. The most common associated symptoms were vaginal bulge, urgency urinary incontinence, and stress urinary incontinence. The severity of the prolapse varied, with more advanced stages observed in the anterior compartment compared to the apical and posterior compartments. Vaginal pessary was the preferred treatment for women with advanced stages of prolapse, while surgical intervention was chosen by those with stage IV prolapse. Voiding difficulty and diabetes mellitus were factors associated with the choice of treatment, with women experiencing voiding difficulty more likely to opt for pessary or surgery instead of pelvic floor muscle exercise only. Age, parity, body mass index, and sexual activity did not impact decision making. Overall, the study highlights the importance of individualized treatment plans that take into account the specific symptoms and preferences of each woman. The findings contribute to a better understanding of pelvic organ prolapse in postmenopausal women and the factors that influence treatment choices.
