Recurrence of Pelvic Organ Prolapse after Anterior Compartment Repair with Mesh - Rodrigo A Cruz, MD

This study aimed to determine the incidence of pelvic organ prolapse recurrence after anterior compartment repair with synthetic mesh and identify potential risk factors for this recurrence. A total of 175 women who underwent surgery for anterior vaginal compartment prolapse were included in the study, with a minimum follow-up period of 12 months. The incidence of local recurrence was found to be 8.9%, with only 2.1% of these women requiring a new surgical procedure. Higher preoperative apical prolapse POP-Q stage was found to be associated with local recurrence of anterior compartment prolapse.<br /><br />Additional findings from the study showed that the incidence of other compartment prolapse was 13%, with apical prolapse being the most common. Among the women with local recurrence, only 2.1% needed a new surgical procedure due to complaints of prolapse. After a multivariate analysis, it was determined that higher preoperative apical prolapse POP-Q stage had a positive association with local recurrence.<br /><br />The overall prevalence of pelvic organ prolapse (POP) ranges from 3% to 50%, with up to 20% of women requiring surgery to correct it during their lifetime. Native tissue repair has high rates of anatomical failure, with a need for re-treatment ranging from 10% to 20%. A systematic review in 2016 found a recurrent prolapse rate of 38% after native tissue repair and a 12.5% risk after transvaginal synthetic mesh repair.<br /><br />In conclusion, this study found that the incidence of pelvic organ prolapse recurrence after anterior compartment repair with synthetic mesh was 8.9%. Preoperative higher apical prolapse POP-Q stage was associated with this recurrence. These findings contribute to the understanding of risk factors for prolapse recurrence and may inform surgical decision-making in the future.
