This study aimed to determine the factors associated with the performance of apical support procedures (ASP) at the time of benign hysterectomy. The researchers analyzed data from 12,345 benign hysterectomies performed within one medical system. They found that 53.6% of patients had concurrent ASP performed, with 46% of them having a diagnosis of prolapse. General obstetricians and gynecologists performed the majority of hysterectomies (67.32%), with 7.09% of them being performed in patients with prolapse.<br /><br />Using logistic regression analysis, the researchers identified several factors associated with the performance of ASP. In patients with a preoperative diagnosis of prolapse, older age, supracervical hysterectomy, and surgeon training were associated with a higher likelihood of ASP. Patients were also more likely to have ASP if their surgery was performed at a teaching institution.<br /><br />The study concluded that even in patients with a preoperative diagnosis of uterovaginal prolapse, ASP are not routinely performed at the time of hysterectomy. The authors suggested that ongoing regional education is needed to raise awareness of the importance of reestablishing apical vaginal support during hysterectomy to prevent incident and recurrent post-hysterectomy vaginal vault prolapse.<br /><br />This study highlights the need for increased awareness and education among healthcare providers on the importance of performing apical support procedures during hysterectomy, especially in patients with a prolapse diagnosis. By implementing these procedures, the incidence of post-hysterectomy vaginal vault prolapse may be reduced.
