Retrospective Assessment of the Long Term Outcome of TVT at 10 years Follows up - Hosam Elhalwagy, FRCOG

This study is a retrospective assessment of the long-term outcome of tension-free vaginal tape (TVT) surgical treatment for stress incontinence. The study analyzed 459 cases of TVT over a 10-year period, with 353 patients receiving TVT abbrevo and 106 patients receiving retropubic TVT. The mean age of the patients was 54.7 years, and the average BMI was 29.<br /><br />The overall success rate at the 10-year follow-up was 379 patients who were continent and satisfied with the outcome of their surgery. When comparing the success rate between TVT abbrevo and retropubic TVT, there was no significant difference. However, 32 patients had postoperative voiding problems and needed catheterization for more than 10 days. Three of these patients needed to return to the operating theater to release the tape. Additionally, 33 patients experienced dyspareunia or groin discomfort, and 18 of them needed tape removal due to erosion/exposure or to relieve groin discomfort.<br /><br />A comparative analysis of postoperative complications did not show any significant difference between TVT abbrevo and retropubic TVT. The study concludes that TVT mesh tapes have an acceptable long-term success rate, and the rate of complications is in line with the Cochrane review. TVT abbrevo has similar adverse outcomes to retropubic TVT and provides the same success rate.<br /><br />Overall, the study supports the effectiveness of TVT as a primary surgical treatment for stress incontinence. However, it highlights the potential for complications such as voiding problems and discomfort, which may require further interventions. Further research and follow-up studies are needed to continue monitoring the long-term outcomes of TVT.
