Single Incision Mesh for Anterior and Apical Prolapse: Does Less Density of Polypropylene Mean Less Complication? - Geronimo A. Bourguignon

This study compares the rate of postoperative complications between two types of transvaginal mesh (TVM) with different densities of polypropylene in patients with anterior and apical pelvic organ prolapse (POP). The study included 151 patients who underwent prolapse repair surgery using a single incision TVM. The two meshes used had different polypropylene densities in the central body.<br /><br />The results showed that the complication rate was similar between the low density group (LDG) and high density group (HDG), with no statistically significant differences. The majority of complications were classified as Dindo Clavien type I and II. There were no differences in the reoperation rate for complications in general between the two groups. However, the incidence of mesh extrusion was higher in the HDG compared to the LDG.<br /><br />Overall, the study suggests that the use of low density polypropylene mesh may have a benefit in reducing the mesh extrusion rate. The demographic and preoperative clinical characteristics of the patients in both groups were similar. The study provides evidence that the density of polypropylene in TVM may have an impact on postoperative complications, specifically the incidence of mesh extrusion.<br /><br />In conclusion, when comparing the rate of postoperative complications between TVM with different densities of polypropylene, the study found that there was no significant difference in overall complication rates. However, the use of low density polypropylene mesh may be associated with a lower incidence of mesh extrusion. Further research is needed to validate these findings and determine the optimal density of polypropylene in TVM for anterior and apical POP repair.
