Skin Closure with Tissue Adhesives vs. Subcuticular Suture after Robotic Urogynecologic Procedures - Sunetris K Fluellen, MD

A randomized control trial was conducted to compare skin closure with tissue adhesives and subcuticular suture in urogynecological robotic surgeries. A total of 50 women were randomized to the study, with 25 in each group. The primary outcome, measured at the 12-week follow-up visit, was the cosmesis of the incision using the Stony Brook Scar Evaluation Scale. The secondary outcomes included length of closure and postoperative wound complications. <br /><br />The study found that the cosmesis score was significantly higher in the tissue adhesives group compared to the suture group. The length of time for closure was also significantly shorter in the tissue adhesive group. However, there was no significant difference in bleeding, dehiscence, infection, or additional wound treatment between the two groups. <br /><br />The study concluded that tissue adhesives result in a superior cosmetic appearance of the scar without increasing the risk of wound complications. They also lead to significantly shorter closure times. Tissue adhesive closure is considered simple, safe, rapid, and bactericidal. <br /><br />It is important to note that the study acknowledges the subjective nature of scar assessment tools and the difficulty in standardizing them. However, cosmesis and patient satisfaction were the main goals of the study. Given the lack of significant difference in wound complications, tissue adhesives were deemed superior as a closure method.
