State-Wide Analysis of Preferred Surgical Approach for Apical Pelvic Organ Prolapse Reconstruction Surgery in Elderly Patients - Sina Mehraban-Far

This study aimed to evaluate the preferred surgical approach for apical pelvic organ prolapse (POP) reconstructive surgery in elderly patients. The Statewide Planning and Research Cooperative System (SPARCS) database was used to identify cases of open and laparoscopic abdominal sacrocolpopexies (ASC) and transvaginal surgeries (TVS) in New York state from 2006 to 2015. Patient age was stratified as young, early-elderly, and late-elderly. The results showed that TVS accounted for a higher percentage of cases in late-elderly patients compared to early-elderly and young patients.<br /><br />Furthermore, the odds of undergoing TVS versus ASC increased by 5.5% with every one-year increase in age for patients over 65. Additionally, a laparoscopic approach was predominantly used for ASC procedures in patients aged 65 and over. <br /><br />The analysis revealed a significant association between advanced age and increased preference for TVS over ASC in the reconstructive POP repair cases. This preference for less invasive surgery may reflect the desire to balance surgical morbidity with lasting POP repair outcomes in elderly patients with multiple comorbidities. <br /><br />The findings encourage further research on POP surgery outcomes in the elderly population. Overall, this study provides insight into the surgical approach for apical POP repair in elderly patients and highlights the importance of considering comorbidities when selecting the appropriate surgical procedure.
