Surgical Outcomes Comparing Patients With Prior Versus Concomitant Hysterectomy Undergoing Minimally Invasive Sacral Colpopexy - Alexandra Dubinskaya, MD

This study aimed to compare surgical outcomes in women who underwent minimally invasive sacral colpopexy with either prior hysterectomy or concomitant hysterectomy. The study included 816 subjects with a mean age of 59.6. The majority of subjects were Caucasian (97.8%) with a mean BMI of 27.4. The majority of subjects had stage III prolapse (69.9%), followed by stage II (23%) and stage IV (6.9%). The mean operative time was 205 minutes.<br /><br />At a median follow-up of 31 weeks, 6.25% of subjects had prolapse recurrence, but the presence of concomitant hysterectomy did not increase the risk of recurrence. Complications occurred in 13.4% of subjects and were more likely with concomitant hysterectomy.<br /><br />The results of the study showed that concomitant hysterectomy was associated with longer operative time but less risk of postoperative complications. The data did not show an impact of concomitant hysterectomy on prolapse recurrence, although longer follow-up may be needed to confirm this finding.<br /><br />The study used logistic regression to assess the impact of prior hysterectomy on prolapse recurrence and major complications. Linear regression was used to assess the impact of prior hysterectomy on operative time. Variables such as age, number of turnovers, and preoperative factors were included in the analysis.<br /><br />In conclusion, this study suggests that concomitant hysterectomy during minimally invasive sacral colpopexy is associated with longer operative time but does not increase the risk of prolapse recurrence. However, it does increase the risk of postoperative complications.
