The Impact of age on Peri-operative Complications after Minimally Invasive Sacrocolpopexy - Kathryn E Barletta, MD, PhD

This study aimed to determine the differences in peri-operative complications after minimally invasive sacrocolpopexy (SCP) based on age. The researchers conducted a retrospective cohort study of women who underwent robotic or laparoscopic SCP at the University of North Carolina between 2006-2016. The women were divided into three groups: older (≥ 65), middle-age (55-64), and younger (< 55) women.<br /><br />The key findings of the study are as follows:<br /><br />1. Women aged 65 and older were not at an increased risk of peri-operative complications after minimally invasive SCP, despite having more preoperative comorbidities.<br /><br />2. Women younger than 55 were at an increased risk of minor complications compared to older women. This may be associated with their higher rate of smoking and concurrent hysterectomy.<br /><br />The study included 441 women, and there were no significant differences in complication rates between the middle-age and older groups. Therefore, the focus of the analysis was on comparing the younger and older groups.<br /><br />Younger women were more likely to be smokers and undergo concurrent hysterectomy, but they had a lower Charlson Comorbidity Index (CCI) compared to older women. The primary outcome showed that younger women had a higher rate of any post-operative complication compared to older women.<br /><br />The most common post-operative complications were urinary tract infections (UTI) and superficial cellulitis, accounting for 60% of all complications. The intra-operative complication rate from SCP was 6.6%, with cystotomy and vaginotomy being the most common intra-operative complications.<br /><br />There were no significant differences in terms of intra-operative complications, re-operation, or re-admission rates between the younger and older groups. However, multivariate regression analysis demonstrated that younger women had a higher rate of minor post-operative complications and UTIs compared to older women, even after controlling for various factors.<br /><br />In conclusion, this study found that older women undergoing minimally invasive SCP were not at an increased risk of peri-operative complications. However, younger women had a higher rate of minor complications, possibly due to their higher rates of smoking and concurrent hysterectomy.
