This study describes the diagnosis, treatment, and outcomes of 19 female patients with urethral diverticula, a rare condition affecting around 0.6 to 6% of women. The management of this condition is challenging due to high recurrence rates and postoperative complications. The objective of the study was to determine whether the use of the Martius labial fat pad graft could improve outcomes. The surgical treatment for all patients consisted of transvaginal diverticulectomy, with the Martius flap used in 14 out of 19 patients. Symptoms at diagnosis included incontinence, pain, prolapse, and urgency. The results showed that the Martius flap, when used after complete excision of the diverticulum, strengthened the anterior vaginal walls and reduced recurrence. Out of the 19 patients, 95% were cured with an accurate diagnosis, proper operation, and the use of the Martius flap. The median age of the patients was 47, and common symptoms included recurrent urinary tract infections, dysuria, dyspareunia, and hematuria. The average operative time was 49 minutes, with an average blood loss of 45ml. After a follow-up of 1.5 years, one patient experienced recurrence, while none developed incontinence or urethral strictures. Pathological analysis showed chronic infection in 52% of samples, one with atypical hyperplasia, and no evidence of removing urethral tissue. Overall, this study highlights the effectiveness of the Martius flap in the surgical management of urethral diverticula in women.
