The Utility of The Australian Pelvic Floor Questionnaire as an Assessment Tool for Urinary Incontinence and Prolapse - Susana Mikhail Mustafa, MD

The study aimed to evaluate the utility of the Australian Pelvic Floor Questionnaire (APFQ) as a diagnostic tool for urinary incontinence and pelvic organ prolapse (POP). A model was developed that included 50 possible predictors, including all the APFQ questions, tabulated scores for different domains, total score, and demographics. The model was used to retrospectively evaluate a cohort of 3502 women, including referrals to a Urogynaecology unit and healthy volunteers.<br /><br />The model accurately identified referred patients with a sensitivity of 89% and healthy volunteers with a specificity of 85%. It had an AUROC of 0.94. The model also accurately differentiated between different pelvic floor dysfunctions, including POP, OAB, and SUI, with varying levels of sensitivity, specificity, and AUROC.<br /><br />The inclusion of condition-specific and qualitative assessments of bladder, bowel, prolapse, and sexual function in the APFQ likely contributes to its diagnostic accuracy compared to other tools. The model was developed using Bootstrap forest and validated using cross-validation. Predictive steps were taken to distinguish between referred patients and volunteers, as well as different PFD diagnoses.<br /><br />The study concluded that the APFQ is a useful diagnostic tool for urinary incontinence and POP. The model developed using the APFQ had high accuracy in distinguishing between healthy volunteers and referred patients, as well as differentiating between common pelvic floor dysfunctions. The relative contribution of predictors to the model was also assessed.<br /><br />Overall, the study demonstrates the potential of the APFQ as a diagnostic tool for pelvic floor dysfunctions and highlights the importance of considering specific and qualitative assessments in such tools.
