Transvaginal Foley Assisted Repair of High Vesicovaginal Fistulas (VVF) for Recurrent Difficult to Manage Cases: A Case Series - Marina Guirguis, MD

This study presents a case series of three surgeries performed between 2005 and 2017 for the repair of difficult-to-manage apical vesicovaginal fistulas using a transvaginal approach. The patients in this series had previously undergone multiple unsuccessful repairs, including abdominal approaches. <br /><br />The technique used in these surgeries involved the use of a transvesical guidewire cannulization and a transvaginal Councill Foley catheter to guide the repair. By applying traction to the fistula using the catheter, direct visualization of the fistula tract was achieved, allowing for successful excision and repair. <br /><br />The results of the surgeries were positive, with successful repair achieved in all three cases. Post-operatively, the patients were followed up, and the first two cases reported no symptoms at their one-year follow-up visits. The third case was asymptomatic at her one-month follow-up visit. <br /><br />In conclusion, this case series demonstrates the utility of using a Councill Foley catheter to assist in the repair of difficult-to-manage vesicovaginal fistulas via a transvaginal approach. This technique proved successful in cases that were otherwise deemed inoperable.
