Treatment of Postpartum Stress Urinary Incontinence in the First 12 months after Delivery: A Systematic Review - Alicia L Gonzales, BSci

Postpartum stress urinary incontinence (PPSUI) is a common disorder resulting from childbirth, with a prevalence as high as 30% at six months postpartum. While SUI during pregnancy often reverses postpartum, residual PPSUI is considered pathological. Current management options for PPSUI include both non-surgical and surgical treatments, with non-surgical interventions traditionally preferred as first-line therapy. This systematic review aimed to describe the efficacy of treatment options compared to controls for curing or treating PPSUI initiated within one year of delivery.<br /><br />The review included seven studies and found that any form of pelvic floor muscle training improved PPSUI. Additionally, at least one session with a physical therapist resulted in greater improvement than home physical therapy alone. However, there is a lack of data to guide surgical counseling in the first 12 months postpartum.<br /><br />The review utilized a search strategy that included MEDLINE and double-screened citations using Abstrackr. Data analysis was performed using SAS 9.4 software, with a one-way ANOVA and post-hoc testing. The risk of bias was determined using GRADE criteria.<br /><br />Based on the included data, all interventions showed improved symptoms compared to the control group. Supervised physical therapy with or without electrical stimulation demonstrated superior results over home physical therapy. Limited data suggested that surgical repair with Burch was also effective in improving PPSUI.<br /><br />In conclusion, this systematic review highlights the efficacy of pelvic floor muscle training and physical therapy for treating PPSUI in the first year after delivery. However, more research is needed to guide surgical interventions for PPSUI during this period.
