Validation of Transvaginal Hysterectomy Surgical Model - Modification of the Flowerpot Model for Vesicovaginal Dissection - Linda S Burkett, MD

The article discusses the validation of a modified Flowerpot model for transvaginal hysterectomy (TVH) surgical simulation. The authors aimed to test the face and construct validity of the modified model and hypothesized that senior trainees would score higher than junior trainees on the Global Rating Scale of Operative Performance (GRS) while performing TVH on the modified Flowerpot model.<br /><br />The study included OBGYN residents and subspecialty fellows divided into a junior group (having completed < 10 TVH) and a senior group (having completed > 10 TVH). The subjects watched an introductory video and then performed TVH on the modified Flowerpot model. Face validity was assessed through a satisfaction survey, while construct validity was evaluated with GRS scores from two expert surgeons.<br /><br />The results showed that all participants were satisfied with the realism of the model, and most were satisfied with the identification of anatomical structures and the vesicovaginal dissection. The majority of subjects were able to identify all pelvic anatomical structures. GRS scores were significantly different between the junior and senior groups, with the senior group scoring higher. The procedure-specific checklist scores did not differ between groups.<br /><br />Overall, the study demonstrates the face and construct validity of the modified Flowerpot model for TVH surgical simulation. The model improved comfort and confidence in performing TVH in the junior group more than the senior group. The authors suggest that simulation training will continue to evolve as a tool for surgical simulation and improving outcomes.
