She also reviewed choosing between ASC and hospital settings, case sequencing, patient optimization, team briefing, standardization, reducing distractions, trainee engagement, and practical positioning tips for vaginal surgery.<br /><br />Dr. Wyman then focused on robotic sacrocolpopexy and how to become less dependent on the OR team by improving personal surgical efficiency. She described a systematic approach to surgery: using efficient instruments, identifying the promontory safely, performing hysterectomy quickly, using a “cut and push” dissection technique for anterior and posterior planes, placing mesh with bidirectional barbed sutures, tunneling for reperitonealization, and using Lasix to improve ureteral efflux during cystoscopy. She also discussed port placement and specimen extraction.<br /><br />During Q&A, both speakers addressed managing idle conversation in the OR, tensioning strategies for sacrocolpopexy, robotic port placement, uterus removal after supracervical hysterectomy, an article on OR pay differences, and which cases are appropriate for ambulatory surgery centers.
